New Patient Registration Process
We’re pleased that you’ve decided to entrust Nashua Medical
Group with your healthcare needs. Our streamlined process makes it fast
and easy to join:
1. Call the Nashua Medical Group Business Office at 891-4450. Please have
your medical insurance card available when you call.
2. We will assign a primary care physician, and answer any questions you
have about Nashua Medical Group. We will also schedule your first appointment.
3. Download, print out and complete the New Patient Registration Form,
which provides us with important information.
4. Read the New Hampshire Patients’ Bill of Rights, then download,
print out and sign the Bill of Rights Acknowledgement Form. This indicates
that you have read the document and understand your rights. If you have
any questions about the form, please contact our Business Office and we’ll
answer your questions.
5. Download, print and complete the Authorization to Release Medical Records
Form and send it to your current physician. This will authorize that doctor’s
office to forward your medical information to Nashua Medical Group.
6. When you arrive for your first appointment with Nashua Medical Group,
please bring your completed forms and medical insurance card. It’s
best to arrive about 20 minutes early so we can review your forms prior
to your appointment.
